Established Patient Communication:

The most efficient way to reach administrative (billing, scheduling, general questions) and clinical staff (medical, prescription, physician questions) in non-emergent situations is through our Patient Portal. Messages left are answered within one (1) business day. Messages left after 12:00pm on Friday will not be answered until the following Monday.



Fees and Payments:

Payment in full is expected at the time of service and can be made with cash, money order, Visa, Mastercard, or Discover. This payment will include any unmet Deductible, Co-Insurance, Co-Payment, or non-covered charges from your insurance company. If you are self-pay or if your insurance coverage is under a pre-existing condition clause or grace period, payment in full for services rendered will be required.


Missed Appointments:

Established patients are required to give a 24-hour notice or you will incur a $25 No Show fee. If you arrive more than 15 minutes past your scheduled time, you will be rescheduled and incur a $25 fee. New patients are required to give a 48-hour notice, or you will incur a $50 No Show fee. If you do not arrive at least 15 minutes before your appointment, you will be rescheduled and lose the $50 deposit. You will be required to submit a new $50 deposit to be rescheduled.


Claim Filing:

Your insurance is a contract between you, your employer, and the insurance company. It is your responsibility to know your specific insurance plan benefits and provisions. You will need to confirm whether we are in-network or out-of-network with your payer. If prior-authorization or referral is required, it is your responsibility to obtain this from your Primary Care Physician. Failure to do so will result in denial of your claims and you will be held responsible for non-covered charges.

 

PLEASE NOTE: Not all services are covered benefits of your plan and may result in fees as non-covered charges. These services include, but are not limited to infertility, contraception, preconception counseling, well woman exams, endometrial ablations, and obstetrical care. You are financially responsible for all non-covered charges.

Annual Exams:

It is your responsibility to know the time frame and amount in which your insurance carrier will cover for annual preventative visits. Additionally, this visit covers (if applicable) your breast exam, pelvic exam, pap smear, and prescription refills. Any secondary problems or procedures may result in additional out of pocket expenses.

Labs:

We do not bill for any services requiring testing outside of our office such as labs, pap smears, pathology, biopsies, etc. In the event you owe anything additional, you will receive a statement from the lab facility resulting these tests for us and will need to speak to them directly regarding any payments, discrepancies, or insurance matters as we do not have access to anything other than results. You may receive a statement from one of the following labs: Arbor Diagnostics, Path Advantage, Copper Creek Health, Clinical Pathology Labs, Medical Pathology Labs, or Natera.


Surgery:

Payment is required in full prior to your scheduled procedure. Our fees do not include an assistant surgeon, use of an outside facility, or anesthesia. Our practice utilizes Baylor Scott and White Centennial, and it is your responsibility to confirm they are contracted with your insurance. Please be advised, the amount we collect upfront is subject to change if any additional services are provided.


Obstetrics:

Once your pregnancy has been confirmed by our office with your initial ‘Confirmation of Pregnancy’ visit, all following antepartum visits will be billed globally to your insurance carrier with your delivery. You will receive both a phone call and a portal message to go over your expected maternity benefits around your 16th week of pregnancy. Throughout the duration of your pregnancy, outside of your Maternity fee, you will only receive bills for ultrasounds, labs, or visits unrelated to your pregnancy (sick visits) depending upon your insurance benefits. Please note, your delivery is generally not billed to your insurance until 30 days later.


Collections:

Outstanding accounts that are not paid within 90 days of receiving your first statement will automatically be sent to our outside collection agency, TSI, and reported to the Credit Bureau. You will also receive a $25 fee added to your balance to cover our costs. Once your account has been sent to TSI, we are unable to see you again until your balance has been paid in full.


Test Results:

Once your performed tests have been resulted, you will receive notifications via phone, text, or email to view these through our patient portal. Messages are attached to each individual result informing you if everything is normal or if you require additional testing. If medication is needed, you will receive notice that this has been sent to your preferred pharmacy.


Family Medical Leave Act and Disability Forms:

If your employer requires paperwork to be completed by your provider, please give us advanced notice. For a 14-day turnaround time, a fee of $25 is required. In the event you need these expedited within 3 days, a fee of $50 is required and due prior to completion. All fees are for each packet requiring a signature. For multiple packets, you will incur multiple fees.


Consent to Treat:

You hereby consent to medical treatment, diagnostic procedures, and injections by providers and staff of Nurture Women’s Health. You understand these testing’s may include but are not limited to labs performed via blood, urine, or tissue. You understand you may be asked to undergo diagnostic radiology procedures including, but not limited to, ultrasounds. You understand you have the right to ask questions about your treatment and you agree to notify your provider of any concerns.
